Chapter 7 Forms for Non-Individual Debtors

 

In addition to the following forms, a List of Creditors is required at the time of filing the petition.

 

  1. The petition PDF should be filed with all the schedules and statements required by 11 U.S.C. § 521(a)(1). The petition PDF file should include the following documents:
    • Voluntary Petition for Non-Individuals (OF Form 201) - Required at case opening.
    • Summary of Assets and Liabilities for Non-Individuals (OF Form 206Sum) - Required at case opening or within 14 days of filing petition.
    • Schedules A/B, D, E/F, G, and H (OF Forms 206A/B - 206H) - Required at case opening or within 14 days of filing petition.
    • Statement of Financial Affairs for Non-Individuals (OF Form 207) - Required at case opening or within 14 days of filing petition.
  2. List of Creditors File - Required at case opening

    Prepare the list of creditors in .txt format. Ensure that New Mexico Taxation and Revenue Department is added to the list, using the address listed below.

The following documents should be filed separately, unless otherwise stated.

 

Required if the debtor is a corporation, other than a governmental unit.

  1. Declaration Under Penalty of Perjury for Non-Individual Debtors (OF Form 202) - Required at case opening or within 14 days of filing petition. Document should be scanned and printed to pdf for uploading as either an attachment to the petition or as a separate document immediately after filing the petition.
  2. Disclosure of Compensation (Director's Form 2030) - Required at case opening or within 14 days of filing petition.s/electronic signature by attorney is acceptable.
  3. Corporate Ownership Statement (NM LF 1007(a)(1)) - Required at case opening
