New required local forms and Clerk’s office forms have been posted to the Court’s website under the red tab labeled, “Forms & Local Rules.” New recommended forms will be added to the Court’s website in the near future. All forms are available in Microsoft Word (doc) and portable document (pdf) formats. The new required forms are simpler and more streamlined than the old forms, and should be easier to use.

The new required forms are:

• NM LF 1002-1 Certification/Statement Regarding Marital Status     Note:  a new event in CM/ECF has been added to use when filing NM 1002-1; the event is available under Bankruptcy > Miscellaneous

• ​NM LF 1007-2​Certificate of Mailing Statement of Social Security Number

• ​NM LF 1009-1(b) Notice of Amendment to Bankruptcy Schedule(s)

• NM LF 1009-1(c) Late Notice to Additional Parties of Bankruptcy Case Filing, Meeting of Creditors, and Deadlines

• ​NM LF 3015-2(c)A Notice of Deadline to Object to Chapter 13 Plan (filed with petition)

• NM LF 3015-2(c)B Notice of Deadline to Object to Chapter 13 Plan (filed after petition date)

• NM LF 4003-1(e) Notice of Amendment to Bankruptcy Schedule C and Notice of Deadline to Object

• ​NM LF 4004-1 Debtor’s Certification in Support of Chapter 13 Discharge

Use of these forms is required by the New Mexico Local Bankruptcy Rules. See NM LBR 9009-1. The forms are numbered to correspond to the related New Mexico Local Bankruptcy Rule. Please begin using the forms immediately.

The bankruptcy software vendors have been notified of these new forms. Nevertheless, if you use a commercial software package to manage the e-filing of documents, please contact your vendor to ensure the required forms are available for use.
