Based on a suggestion from a practitioner, the clerk’s office has revised Local Form 902, the Omnibus Signature Page and Declaration, to incorporate the certification regarding marital status.  Petition filers who use the new form will no longer have to have debtors sign the marital status form separately.  The revised form is posted on the Court’s web site.
