On August 29,2011, the court entered an order adopting revisions to Local Rule 1006-1. The rule has been revised to reflect that local forms NM OF 3A and NM OF 3B are available as an option for compliance with the rule, but are not required. The national official forms 3A and 3B are also accepted by the Clerk’s Office.

Practitioners should also note that Judge Starzynski has revised the pretrial conference forms for his chambers. The revised forms are available on the Judge's web site under the section titled "Form Orders for Adversary Proceedings."
